4019 sometimes doesn't start after patch R8.0.1 35h inst

Customer used R8.0.1 patch 9. It was installed patch 35h. Some x9 sets ( 4019 today ) doesn't start on some board or CH IP shelf reset.
set 4019 shows 3.81.00 on the screen, it means that binary upgraded according TC1434.
It wasn't such problem before according customer info. 4019 works after reset by termstat.
No linked incidents. All others 15 sets had started on UA16 startup apart this one.
Had somebody experienced such bug?
