incoming number& name appearing twice

Hi , I have an OXO with 2 attendants. The attendants phone are 4029.
On one attendant phone i am having an issue ,whereby the incoming call number/ name of the caller is appearing twice while it is still ringing.
I have unplugged & replugged the 4029.Still same issue.
I copied all the keys and features of the 2nd operator on the 1st operator: still same issue.
Any ideas?
