telpatch not running

i am having a problem with R5.0Ux-d2.313-7-n-ng-c6s2

when the cpu is booting it fails to load telephone application.
the console output is as follows:-

what could be the problem? to start the system i have to shutdown, remove the cpu and then start again after some time. if i switch cpu off and then start, it is giving the same error.

Do you have a redundancy CPU configuration ?
I would reinstall the patches first, and if it is not working, reinstall everything. Did you change anything before you got this error ?
