Hold and Conference Issue

Hi All ,
This is my first post related to my first OXO installation as well .
OXO is installed and configured with a SIP-Trunk to an ITSP, i've two issues and i think both are kinda related
1- 3ways Conference calls internally ( phone to phone ) are fine , als if 2 phones and one Mobile ( External call ) its ok but when calling 1st mobile and then trying to add one more mobile ( 2 external parties ) to the conference it is showing Conference rej
2- the second problem is when putting an External ( incoming call ) on hold is dropping the call , not the case if its an External (outgoing call )
means when the user receive an external call and puts it on hold it gets dropped but if the user is the one making the external call its working fine
Any ideas ?
