Digital phones 4039 " ** 4039 V3.80.90 ** issues

Hello All,
I have OXE in Release 8.01 ( i know, it's an old version, i'm upgrading soon,,,)
recently, i'm noticing some bizarre behaviour on some digital extensions that are not coming into service after a double bascul.
when i do a "listerm Cryst board", the extension is shown as been in service, and event by going in mgr, user/dynamic state user, user is seens by OXE as been in service, and even when you call that user from another OXE's extension, you can hear a ring back tone as if that extension was ringing correctly before overflowing to the VM.
the reality is that, the phone is stuck in initialization mode, and the diaplay is showing " ** 4039 V3.80.90 **
I resetted the port many times, for some extension they are coming up after a reset and some are not, we have to change equipment address to have them come into service again.
Question: is it something that you have already noticed? is this a sign of bug that may be fixed in the newer release ? please advise.
