Screened transfers recalling/failing intermittently

I've scouered through the various boards and can't locate a similar issue, sorry if it's redundant question.
R9.1 w/GCE 1.0 (I've been able to reproduce with non-agent phones, too).
1) Calling party via PRA to agent, is answered ok.
2) Agent presses 'transfer' feature softkey or F2, then searches for tranfer destination via Dial By Name
3) Agent selects destination
4) Called party answers, agent screens and transfers the calling party, then releases.
Destination is internal extension, not forwarded, has associate for VM.
Here is the rub: Most of the time the call will transfer ok, other times not (to the same destination). Has the appearance of 'recalling' to the agent before the called party answers. No other status message from the called station (busy/dnd, etc.). Very intermittant.
Not sure if there is any correlation but I'm seeing compressor errors - possibly near the time of the failed transfer. Also seeing some MDCU errors, but nothing major.
Any suggestions as where to continue digging would be greatly appreciated!
