One Way Audio and IP soft phone drops

I have a customer that is experiencing one way audio on IP sets and and IP soft phone disconnects, the site is running i160533c (9.1). The LAN/WAN staff say that they are set up for QOS and all the ethernet ports are set to 100 Mbs Full Duplex.
I've checked the PCX and can find no mismanaged parameters.
Does any one have any suggestions??
