Who's the carrier providing dial tone? What protocol are they using on the trunk group? NI2?
What are your trunk group settings??? Is this in the US?
What do you get when you run infocs command?
Have you reloaded software and reloaded the database?
Sorry, lots of questions, but normally upgrade or software reload fixes the problem 99.999% of the time, you know that's where they get the 5 nines rule.

This is in the US and we have confirmed that we are setup the exact same way as the carrier and everything is programmed correctly for the carrier on their side and ours. I have applied 9 patches and a full upgrade from 7.1 to 8.0.1 and still no resolution.

Did you tried to use some analog public lines just for testing purposes?

Have not tried that and thought about it until I saw the system fail when dialing internally 4 digit exts. I have users who are saying when they dial someone's ext internally...they are being routed to a totally different ext number! WTF!

Your database is screwed. I would reload software and start from scratch, not a good answer, but the best solution.

Maybe can try installing a 3rd party Auto attendant system...if it works...considering throwing away the internal AA

Can't see that helping tritel as AA is not used when dialing extn to extn.
