the solution was by changing the coupler for the fiber and complete system reboot (not bascul) it was working fine.

I currently have a problem at customer site with shelf not coming into service. Issue seems to be problem for a long time as no cards are in service and it shelf is running without any cards on it.
Installation:
Site has 3 ACT 28 shelves i.e. Shelf 0, 1 and 2. INTOFs are connect from back plane using TY2 cables.
Main Shelf 0 has 3 x INTOF A cards ( 0-12, 0-15, and 0-24) all in service.
Shelf 1 has 2 x INTOF B configured in slot 6 (main) (remote 0-24)and slot 20 (standby) (remote 0-15) both working fine in service with 78 time slots.
Shelf 2 has 1 x INTOF B configured in slot 20 (remote 0-12). I am unable to get the INTOF B (2-20) side of it working. It looks like card 0-12 and 2-20 can see each other but 2-20 is unable to come into service within the software.
I have tried the following to try and resolve issue but unsuccessful.
1. Swapped with replacement INTOF cards (a tested working card in shelf 1). Even tried with INTOF2A/B cards after reconfiguration.
2. Swapped with another TY2 cable that works fine to link shelf 0 and shelf 1 INTOF cards.
3. Reconfigured ACT 28 shelf 2 after deletion.
4. Reconfigured INTOF B position from slot 2-20 to 2-6 but still no good.
5. Configured eUA and Z cards on shelf 2 and pair along with the INTOFB just incase the shelf was unable to come into service without any Analog/Digitals cards attached.
6. Swapped an INTOF A with an INTOF B just incase of incorrect binary or difficulty in downloading binaries.
7. Check the synchronisation - NPRAE accesses Synchronisation is 10 and 20 respectively.
The only thing remaining to test as I have exhausted all avenues is to try a another ACT 28 shelf perhaps.
The incidents shown are:
000001M:000/12/-/---|=0:3660=status of 8KFS (0 is OK, 1 KO): 0 , value : 5 5
The site has been upgraded to latest R11.1 software and also the CPU6's been replacement with CPU7-2. Seems like the shelf never worked in the past either and cards from this shelf were relocated onto shelf 0 and 1. I would love to get this sorted.

try to reset the intof A , check if there is no alarm in the npra' clock source '. 10 and 20 are at MHO to much high if any trouble it will repecute in intof : i have seen that not only on core node( master node( but on all networked nodes all intof on remote nodes become not synch lol
always when we perform upgrade we download all intof bin one by one on a position at the main shelf .

also check the intof slot pins may be one is broken
