PCM2 ON CRYSTAL ISSUE

Hi Gurus
my colleague try to install a PCM2 for OXE-CRYSTAL
He has created the board on position act 0 board position 18 with default parameters
after plugging the board he perform cplstat 0 -18 , config 0-18 etc shown INIT 1 RUN ? AND INIT 2 RUN , but after that he see board Out Of Service with cpu led fixed
on omnivista /alarm he see start and the end of bin downloading and board out of serve . i ask him to unplug/plug a second time the board and perform incout -3 to see real time incidents and has seen the 3001 alarm index: io1 trans mission error 1 4 18 0 0
any help is welcomed and thanx in advance

looks like a hardware issue. how is the board declared?
