There a Techtip regarding flash Errors on 6450 (and 6250).
Techtips are 12505 (for 6450) and 12506 (for 6250)

Release note states the bug was fixed in 6.6.3 451.

Jorge described the necessary steps to recover..

Hi..

I saw one post in unleased that , to issue command "setNextRunningVersion 2" and "reboot" and it is rebooting into working.

after that i got login console.. and logged in with default usernam and passoword.

then i checked directories as shown below.

ls certified

drw 2048 Feb 3 2000 ./
drw 2048 Feb 3 2000 ../
?-- 0 Jan 1 1970 KFbase.img
-rw 2325506 Dec 31 2000 KFdiag.img
-rw 5083931 Dec 31 2000 KFeni.img
-rw 2511585 Dec 31 2000 KFos.img
-rw 597382 Dec 31 2000 KFsecu.img
-rw 705 Dec 31 2000 software.lsm
-rw 2025 Feb 3 2000 boot.cfg

59908096 bytes free

-> ls working

Listing Directory /flash/working:

drw 2048 Feb 3 2000 ./
drw 2048 Feb 3 2000 ../
-rw 15510736 Dec 31 2000 KFbase.img
-rw 2325506 Dec 31 2000 KFdiag.img
-rw 5083931 Dec 31 2000 KFeni.img
-rw 2511585 Dec 31 2000 KFos.img
-rw 597382 Dec 31 2000 KFsecu.img
-rw 705 Dec 31 2000 software.lsm

59908096 bytes free

-------------
BUt i after this i checked flash.. ( -> fsck /flash) and found errors as below..

Error found in inode tree of the following:
/certified/KFbase.img
Results of check on all directory contents and inodes.........ERROR

Errors Detected:
Invalid Inode Meta Block Encountered.

Volume Information:

My concern on this issue is , is reimage is still necessory ( even if this error persist ) , as it is booting to configuration console..

kindly note that .. KFbase 's size in certified mode is 0 ( is this error..)

Dear Hey Jorge Matias and Devnull .. kindly help me on this ..

Dear Hey Jorge Matias and Devnull .. kindly help me on this ..
At this point you don't want to try to recover filesystem, because data in flash is irrecoverable! You need to format the filesystem and install a freshly new firmware!

Follow the instructions I have written in my previous post, which includes the flash formatting.
That will bring your switch into a properly working condition again.

Jorge Matias

If you don't recover now you will have the same error after a reboot (Working and certified differ -> hence it boots certified with the 0 byte base.img)

-> recreate flash and recover with a current release.

If you copy your boot.cfg into your recovery USB Stick it will copy that as well, so you don't have to reenter the whole configuration. You'll still need the UserTable, banners aso..

Are you able to do a "ls" command in kernel mode?
Are you able to view Crash.pmd file?

Thanks for this post! I just followed the procedure indicated by Jorge Matias and it worked smoothly on a OS6250-24.
After power cycling the switch i copied the images to /flash/working directory, and executed "reload working no rollback timeout" so i
could configure the switch and save changes.
-write memory
-copy working certified.

Enrique
