I have an OS6250 switch that is always to restart endlessly.
After I turn on the console, could interrupt the process and enter
[Miniboot] ->
How do I copy / reinstall the firmware and put it to work normally?

What does it say when it reloads in a loop?
several ways to recover:
if you have a disaster recovery usb stick it should be enough to plug it in, the switch will get its images..
see viewtopic.php?f=394&t=21093 for more info.
otherwise try to see what exactly is wrong (is an image corrupted? missing?)
ll "working"
ll "certfified"
if certified is empty or missing some files but they are present in working you can copy them over from working:
cp "/flash/working/KFos.img","/flash/certified"
if you have an kbase.img with a size of 0 it seems you have hit a bug that was present in past versions.
then prepare an USB Stick do a
sysNewfs "/flash"
and use the usb stick.
if you don't have a stick you can also use zModem or tftp from uboot/miniboot, but this is really cryptic..

i have a switch OS6520, my device was lost all *.img in the folder certified. When device start, it runs on miniboot, i have tried to reload image by usb but miniboot did not found the usb (my usb format FAT, size 1GB)
Can you guide me how to switch reloaded os for my switch, I do not understand how to load os with xmodem, you can guide me.

If you have another switch:
Try different USB Sticks. just type usb enable and see whether it recognizes the sticks.
Other:
If you still have your images in /flash/working you can copy the over from miniboot as i have written in the first post.
I never tried XModem, but recoverd a switch using zModem. this realy takes time..
It was: (if i remember right):
- cd "certified"
- sysModifyBootParameters
- boot serialbaudrate 115200
- commit system
(baudrate to 115200 otherwise it takes ages - remember you have to change your console port on the laptop afterwards as well)
do not use commit file, otherwise this will be stored in boot.params..
Have your files ready for Transfer with zModem (i use SecureCRT, but other programs should work as well)

Should take about 30-40 Minutes

good.. now get various sub sticks an test whether they are recognized.. reduces recovering time to about a minute or so in the future.

I don't know if you have already solved your problem, but what you should do is:
Step 1)Prepare the an USB Pendisk (2GB or less) to recover your switch. Use the following steps
Step 1.1) Format an USB Pendrive (2GB or less) with FAT16 filesystem in your computer
Step 1.2) Create directories in USB Pen's filesystem
/6450
/6450/working
/6450/certified
Step 1.3) Download the switch firmware archive from Alcatel and extract all files, especially the *.img files, and copy them to /6450/certified directory in USB Pen's filesystem
Step 2) Boot the Switch and wait until reaches the fallback prompt with mentioned earlier, which is "Certified: [Kernel]->"
Step 3) Format the switch's flash executing
Certified: [Kernel]-> sysNewfs "/flash"
Step 4) Insert the USB Pen in the switch's USB port
Step 5) Power cycle the Switch and allow it to automatically copy the firmware from the USB Pendisk.
You can read more information from "OmniSwitch 6250/6450 Switch Management Guide", Chapter 5, in topic "Disaster Recovery Using USB".
